Community Health Worker Program

The Community Health Worker program is a component of the Outreach and Education elements of CHOICE’s core mission. Our Community Health Workers (CHW) are hired from within the local community and perform non-clinical health-related services. They help our patients through outreach to spread awareness of available health services and programs, provide assistance with enrollment in assistance programs such as Medi-Cal, as well as options for those who do not qualify, conduct screenings for common health concerns such as hypertension and diabetes, coordinate care and follow-ups to encourage treatment compliance, health education, and connection to mental health care and substance use recovery services.

Our Community Health Workers have roots within the community and have a deep desire to help improve the quality of life for their neighbors. Many of them have experienced the same health concerns that our patients encounter, which fueled their passion to be Community Health Workers. This puts them in a unique position of not just educating the public on health topics but sharing real personal experiences and lessons learned,. Being able to relate with the patient leads to meaningful relationships and has a longer lasting impact on our patients’ choices in regard to their health.
