Community Health Workers (CHWs) are trusted members of the community who work one-on-one with individuals and families to remove barriers to care, provide support, and connect them to the resources they need.

WHAT WE DO

Meeting people where they are.

Our Community Health Workers help identify needs, provide education, and connect individuals and families to services that support their overall health and well-being.

✔️ Health education and resource navigation

✔️ Assistance with accessing medical, behavioral, and social services

✔️ Support with enrollment in programs (e.g. Medi-cal, CalFresh)

✔️ Help with overcoming barriers to care

✔️ Culturally and linguistically appropriate support

✔️ Ongoing follow-up and encouragement
