Primary Care
We offer comprehensive primary medical care services that help prevent, diagnose, and treat acute and chronic conditions. Our team of health care providers supported with diagnostic laboratory and radiologic services is ready to help provide top-quality service to all of your family members.
Pediatric Care
Our health care professionals can enact a plan to lay down a solid foundation to support your children’s health and wellbeing. We’ll monitor your child’s development with regular physical exams and keep them up to date with immunizations so that you can feel more comfortable as they explore the world.
Community Health Worker Program
Our Community Health Worker Program is a critical component of the Outreach and Education elements of CHOICE’s core mission. Having workers from within the communities we serve who have experienced the same health events as our patients put us in a unique position to build rapport, comfort, and trust. Those elements lead to excellent and long-lasting treatment compliance and healthier lifestyle choices thanks to the health education program.
Integrated Behavioral Health Services
For Integrated Behavioral Healthcare, we combine medical treatment and the management of related behavioral health issues within a single healthcare facility.
